Your Next Step: Complete The Cohort Intake Form

Thank you for your leadership in bringing this exclusive cohort together. I am truly honored that you have chosen me to guide your group on this journey.

The first step in our onboarding process is for you to complete the intake form below for each member of your cohort. This is a critical step that allows me to gather the necessary information for everyone and set up the program internally.

Here is a quick overview of what to expect:

1. You Complete the Form: After you submit the intake form for a person, they will automatically receive a welcome email.

2. They Register & Pay: Within 48 hours, they will receive another email with a link to register and pay for the program.

3. They Complete Their Assessment: After each person has completed their payment, they will receive a separate email with a link to their personal coaching assessment.

It is important that this step is completed promptly, as we cannot set our recurring weekly schedule and begin our work together until all intake forms are submitted. The sooner we get this done, the sooner we can begin the journey to your team's breakthroughs.
